MIRACUM DIFUTURE Alignment Hub
What is it all about?
The MIDIA Hub aims to pilot and sustainably implement digital solutions for cross-sector, patient-centered care in real-world settings. Clinical use cases include prehabilitation for patients with gynecological tumors and the care of patients with multiple sclerosis. Communication between general practitioners, specialized outpatient practices, hospitals, and university medical centers is to be structured and improved through digital platforms for physicians and patients.
How does the study work?
Digital portals are co-designed and tested together with physicians, patients, and the company Monks – Ärzte im Netz GmbH. In the neurological use case, the practice-based research network BayFoNet is integrated in a systematic way, enabling digital support of the entire patient journey—from general practice to specialized neurology care and on to university-level tertiary care.´In the gynecological domain, a prehabilitation app is used to motivate patients to engage in targeted interventions in the weeks prior to planned surgery. National infrastructures such as the electronic patient record and the telematics infrastructure are integrated and evaluated with regard to their usability.
